Hold Harmless Agreement
I agree to obey all rules and regulations of Ninpiden Dojo and all other schools of the federation which are established for the purpose of keeping the order of the organization and to reduce the potential for injury,
I acknowledge and hereby release and hold harmless the above organization, its officers, members, instructors, employees, and any other authorized invited guests or observers from any and all claims for injury to person or property (whether or not said injuries are immediately apparent upon occurrence), which I may receive while participating or training in the activities of the Ninpiden Dojo, its training programs or its distance learning programs, through the use of any facilities and equipment connected herewith.
I in consideration of your audio recording, photographing or videotaping me, hereby grant Ninpiden Dojo and its employees, agents and assigns, the right to use, edit, publish, and/or display photographs, audio recordings, or videotaped images of me in the promotional work of Ninpiden Dojo in all media.
I agree to hold Ninpiden Dojo and its employees, agents and assigns, harmless against any possible liability resulting from the use of photographs, audio recordings and videotaped images of me, and I hereby release and discharge Ninpiden Dojo and its employees, agents and assigns from any claims whatsoever in connection with the use of such media.
I hereby certify that I have read the foregoing and fully understand its meaning and effect.
I agree the release freely and voluntarily and in executing this release do not rely on any inducements, promises or representations made by Ninpiden Dojo or its employees, agents and assigns. I understand that by participating in this program I am accepting these conditions, and agree to abide by them.
